Family Guide on Returning to School

• Talk to your children about what school will be like when they return.
• Ask if they have questions and let them know we are all going to work together to keep everyone safe and healthy.
• Practice wearing a mask or face covering during the day.
• Discuss the importance of social distancing.
• Talk about washing hands and covering coughs and sneezes.

 Required Forms

• Health Information Form (HIF) (SS/SE -71)
• Emergency Care Information form
• Both forms are required/mandatory for all students for SY 20-21.
• You can bring forms to supply pick up, mail forms to the main office, or drop them off anytime between 9:00 – 1:00 each day.

• Parents and guardians, please review the Student Rights and Responsibilities and related resources at www.fcps.edu/srr.  After reviewing the information, fill out the SR&R Parent Acknowledgement Form using this LINK we have provided.  The form must be filled out one time per student.
